gallium: Add SWITCH, CASE, DEFAULT and ENDSWITCH opcodes to TGSI.

Provide reference implementation of them in tgsi_exec.

Note that BREAK opcode is overloaded and can be used to break out
of either a loop or a switch-case statement.
